Care Manager RN

At HealthCare Talent, we are seeking a highly skilled Care Manager RN to join our team. As a Care Manager RN, you will be responsible for working effectively with our Integrated Care Team to provide high-quality, effective care management to our members.
Key Responsibilities:
- Conduct comprehensive, holistic assessments of members' medical needs.
- Develop and implement individualized care plans (ICPs) to address members' needs.
- Communicate ICPs with members, their families, and other members of the care team.
- Lead interdisciplinary care team meetings to share information and update care plans.
- Coordinate with internal and external health partners to support members' comprehensive care needs.
Requirements:
- Minimum two (2) years of clinical experience in an acute care facility, skilled nursing facility, home health, or clinic setting.
- Possession of an Active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN.
- Two (2) or more years of care management experience in a healthcare delivery setting preferred.
