Job Summary:
Brightshores is seeking a highly skilled Nurse Practitioner to join our team in the Grey Bruce Diabetes Program. As a key member of our interdisciplinary healthcare team, you will provide advanced practice nursing level care to patients at risk for or coping with diabetes and related chronic illness.
Key Responsibilities:
- Deliver high-quality patient care in accordance with Diabetes Canada Clinical Practice Guidelines and the Standards for Diabetes Education in Canada.
- Collaborate with the program team and other care providers to develop and implement comprehensive care plans.
- Provide clinical leadership and support to the Diabetes Program Medical Director and other healthcare professionals.
- Participate in the development and evaluation of services related to diabetes care.
- Lead the NP-Led Diabetic Foot Ulcer Clinic and provide clinical leadership within the team and to the broader healthcare community.
Requirements:
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (EC).
- Completion of the Nurse Practitioner Program, Primary Care or Adult.
- A minimum of 2 years related experience with chronic disease care, with a preference for recent experience in diabetes care.
- Certified Diabetes Educator or willingness to obtain within 1 year of employment and maintain certification.
- Member of the Diabetes Educator Section of Canadian Diabetes Association.
- Sound clinical background and strong health assessment and diagnostic skills.
- Demonstrated ability to work collaboratively as part of an interdisciplinary healthcare team.
- Excellent interpersonal, communication, and leadership skills.
- Excellent organizational and critical thinking skills.
- Self-directed with demonstrated creativity, flexibility, and initiative skills.
- Demonstrated experience in collaborative development of clinical service programs.
- Knowledge and experience utilizing the principles of adult learning and chronic disease self-management.
- Experience working within/or awareness of Aboriginal culture.
- Valid Driver's license and access to a reliable insurable vehicle.
- Ability to travel to venues outside of assigned site.
- Availability to work evenings and weekends to accommodate patient and program needs.
- Recent satisfactory attendance and performance records.
- Adherence to Brightshores' CARE Behaviours (Collaboration, Accountability, Respect, Excellence) and LEAD Competencies (Lead by example, Empower, Achieve results, Develop others).
- Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at Brightshores.
