Job Summary
We are looking for a dynamic and compassionate individual who can provide intensive individual and group therapy, liaison services, and care coordination support to clients across the care continuum. As a key member of our team, you will work collaboratively with other healthcare professionals to ensure delivery of consistent, relevant, and timely treatment, rapid re-engagement, and access to information where appropriate.
