Job Summary

We are seeking a highly skilled Senior Geriatric Nurse Practitioner to join our interdisciplinary team at Bayshore HealthCare. This dynamic role requires a passionate and dedicated individual who can provide exceptional primary care to patients, focusing on health promotion and maximizing patient safety and function.

The ideal candidate will have a minimum of 3 years of geriatric experience and be able to prescribe controlled substances as authorized by the College of Nurses of Ontario. A Master of Nursing degree and current Extended Class certification with the College of Nurses of Ontario are also required.

About the Role
  • Provide comprehensive primary care to patients as part of an interdisciplinary/interprofessional team focused on creating a person-centered plan of care
  • Support the needs of patient populations through innovative approaches to care delivery
  • Promote evidence-based practice through the use of latest research-based guidelines
  • Ensure the provision of safe, evidence-based, cost-effective, patient- and family-centered care
Requirements
  • Completion of a recognized Primary Care or Adult Nurse Practitioner program
  • Able to prescribe controlled substances as authorized by the College of Nurses of Ontario
  • Master of Nursing degree
  • Current Extended Class certification with the College of Nurses of Ontario
