Nurse in Community Setting

Overview
CarePartners is seeking a skilled Community Nurse to join our team in the Southwest, Durham, On area. As a Community Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinic, retirement home, or school, providing exceptional nursing services to patients.
Key Responsibilities
- Develop and maintain effective Care Plans with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
- Work with patients to provide exceptional nursing services, both in-person and virtually.
- Communicate and liaise with patients, family members, and others within the interdisciplinary care team.
- Perform other clinical duties as required.
Requirements
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma.
- Current College of Nurses of Ontario (CNO) registration in good standing.
- Strong desire and commitment to making a difference in the lives of our patients.
- Exceptional interpersonal skills.
- Valid driver's license and vehicle, as regular travel is required.
- Excellent English verbal and written communication skills.
- CPR Certification.
- Current immunization record, including 2-step TB test.
- To protect our patients, we require a current vulnerable sector check and a clear background check.
