Job Summary

The successful candidate will be responsible for providing comprehensive nursing care to clients in their homes and communities. This includes assessing client needs, developing and implementing individualized care plans, and evaluating outcomes. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently and collaboratively as part of a multidisciplinary team.

Key Responsibilities
  • Provide holistic nursing care to clients, including physical, emotional, and social assessments.
  • Develop and implement individualized care plans that address client needs and goals.
  • Evaluate client outcomes and adjust care plans as needed.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
  • Communicate effectively with clients, families, and caregivers to promote understanding and engagement in care.
Requirements
  • Degree in Nursing (Bachelor's or Master's) from an accredited program.
  • Current registration as a Registered Nurse (RN) with a province-specific regulatory body.
  • Minimum 2 years of experience in community health nursing, preferably in a similar role.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Able to work independently and collaboratively as part of a team.
  • Familiarity with quality improvement principles and practices.
  • Ability to adapt to changing situations and priorities.
Salary Range

$75,000 - $90,000 per year, depending on experience.
