Job Title: Personal Support Worker - Part Time

At Southbridge Care Homes, we are seeking a compassionate and dedicated Personal Support Worker to join our team on a part-time basis.

Job Summary:

We are looking for a skilled and caring individual to provide direct care and support to our residents. As a Personal Support Worker, you will work collaboratively with our multi-disciplinary team to meet the needs of our residents and provide a high level of care and support.

Key Responsibilities:
  • Provide direct care and support to residents, including assistance with daily living activities.
  • Work with the care planning team to develop and implement individualized care plans.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care delivery.
  • Participate in ongoing education and training to enhance your skills and knowledge.
Requirements:
  • PSW Program Certificate or Diploma that meets the appropriate standards established by the Ministry of Training, Colleges, and Universities, the National Association of Career Colleges, or the Ontario Community Support Association.
  • Passion and heart for providing quality care and support to residents.
  • Knowledge and experience in care planning systems and documentation.
  • Excellent communication and interpersonal skills.
