Job DescriptionThe Care Management Coordinator plays a critical role in facilitating patient care and discharge planning. This individual will collaborate with the Patient Care Coordinator, physician, and interprofessional team to determine discharge goals and prioritize discharge planning activities based on patient level of need/risk.
This position requires strong communication and problem-solving skills, as well as the ability to work independently and collaboratively within an interprofessional team. The ideal candidate will have comprehensive knowledge of nursing theory and practice within a patient/family centered model of care.
Responsibilities- Coordinate care and discharge planning for patients, working closely with the Patient Care Coordinator and interprofessional team to determine discharge goals.
- Facilitate patient flow activities by identifying patients requiring specialized attention and coordinating patient transfer to alternate level of care.
- Observe, monitor, and evaluate assigned patient progress, symptoms, and behavioral changes, intervening to facilitate resolution of problems and removal of barriers.
The successful candidate will possess current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) and three years recent, related acute care experience including one year of experience in community care and discharge planning.
A broad knowledge of evidence-based nursing practice, clinical pathways, expected length of stays, resource utilization, and patient assessment is also required. Additionally, the ability to engage in comprehensive assessment, observation, and monitoring of patients, as well as plan, organize, and prioritize work is essential.
