About the Role

We are seeking an experienced Family Physician to join our interprofessional team at Somerset West Community Health Centre. As a Primary Care Physician, you will provide comprehensive primary care to our clients during acute and chronic illness. You will work collaboratively with a diverse team of healthcare professionals to develop, implement, and evaluate programs and services that cater to the unique needs of our clients.

Responsibilities

  1. Provide high-quality, patient-centered primary care to individuals and families within our community.
  2. Participate in the development, implementation, and evaluation of programs and services that promote health and well-being.
  3. Collaborate with an interprofessional team to deliver comprehensive care that addresses the physical, emotional, and social needs of our clients.
  4. Prioritize cultural safety and adapt your practice to meet the diverse needs of our multicultural community.
  5. Engage in ongoing professional development to stay current with best practices in primary care and community health.
  6. Contribute to the growth and development of our team through mentoring, education, and quality improvement initiatives.

Requirements

  • Certification by the College of Family Physicians of Canada (CCFP) and licensure as a Family Physician in good standing with the College of Physicians and Surgeons of Ontario.
  • At least 3 years of experience in primary care, preferably in a community health setting.
  • Demonstrated knowledge and understanding of the Community Health Centre model of health and well-being.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and teams.
  • Ability to provide service in multiple languages, including English and French.
  • Familiarity with electronic documentation systems and proficiency in using technology to support clinical practice.

We offer a competitive salary range of $176,718 - $204,675 per annum, plus an on-call stipend.
