Job Description

The Nurse Navigator - Community Mental Health position is responsible for providing clinical assessment, case management, and interventions to support individuals experiencing mental health and addiction issues. This role involves working with clients and their families to address emotional and social needs, with the goal of promoting social functioning and overall well-being.

Key Responsibilities:

  • Conduct comprehensive assessments to identify client needs and develop individualized care plans
  • Provide case management and coordination of services to support clients in achieving their goals
  • Deliver therapeutic interventions and support to clients and their families
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support
  • Develop and maintain relationships with community resources and services to support client needs
