Job Title: Registered Nurse

ComForCare Home Care is a leading provider of in-home care services. We are seeking a skilled and compassionate Registered Nurse to join our team.

About the Role:

As a Registered Nurse with ComForCare, you will be responsible for conducting thorough client assessments, creating personalized care plans, implementing interventions to achieve client goals, and updating care plans to track progress. You will also administer medications as needed, maintain detailed client records, and coordinate referrals to other healthcare professionals as required.

Responsibilities:
  • Perform comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
  • Develop an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
  • Provide interventions with a focus on achieving realistic client outcomes.
  • Report changes to update the plan of care to reflect progress towards goals and outcomes.
  • Administer medications as needed.
  • Maintain client records showing systematic assessments, planning interventions, and evaluations.
  • Initiate referrals as needed to other health team members.
Requirements:
  • Graduate of, at a minimum, a Practical Nursing Program (2-year college certificate)
  • Registered with the College of Nurses
  • Minimum of one (1) year experience in an acute care setting or equivalent experience
  • CPR certified
  • Demonstrated written, verbal, and interpersonal communication skills
  • Licensed driver with automobile insured in accordance with provincial and/or Agency requirements
