Pharmacist Leader

We are seeking an experienced Clinical Pharmacist to lead our Pharmacy Care Clinic team at Rexall Pharmacy Group ULC. As a Clinical Pharmacist, you will be responsible for delivering exceptional patient care services, working closely with healthcare professionals to provide prescribing services, and promoting pharmacy initiatives to achieve desired health outcomes.

• Manage and oversee the operations of the Pharmacy Care Clinic, ensuring seamless delivery of services to patients.
• Complete comprehensive patient assessments and prescribe new medications or extend existing ones as needed, in accordance with provincial regulations.
• Provide Travel Health Consultations, Chronic Disease Management, and conduct annual Patient Medication Reviews, with follow-ups as required.
• Administer medications by injection, perform Point of Care screening/swabs, and provide lab requisitions and interpret lab results (subject to provincial legislation).
• Collaborate with other Healthcare Professionals to establish circles of care for patients and counsel them on prescription medication use.
• Support dispensary activity and pharmacy workflow, collaborating with other healthcare providers to maintain continuity of care.

• Bachelor of Science in Pharmacy, fully licensed in the Province of employment and in good standing.
• Injection Certification is mandatory; additional accreditations are an asset.
• Must have Prescribing Authorization.
• Excellent organizational, problem-solving, and decision-making skills, with effective planning and time management abilities.
