Job Summary
We are seeking a highly skilled and compassionate Care Navigator to join our team. As a Care Navigator, you will play a vital role in assessing the needs of our clients and connecting them with the necessary resources and support services.
Key Responsibilities
- Develop and implement individualized care plans that meet the unique needs of each client.
- Provide guidance and support to clients and their caregivers to navigate the healthcare system.
- Connect clients and caregivers to community resources and services, including funding sources.
- Conduct thorough assessments to identify client needs and develop strategies to address them.
- Provide crisis intervention and monitoring services to ensure client safety and well-being.
Requirements
- Registration as a regulated health care professional, including registered nurse, physiotherapist, occupational therapist, speech language pathologist, or social worker.
- Minimum of one year of experience in the community support service sector or healthcare environment.
- Experience working with frail and elderly populations, as well as knowledge of community resources and funding sources.
- Strong case management and coordination skills, including assessment, scheduling, and implementation of services.
- Excellent computer skills, including proficiency in MS Word, Outlook, and PowerPoint.
- Valid Ontario Driver's License and access to a reliable vehicle.
- Valid Vulnerable Police Record check completed within the last 12 months.
- Current CPR/First Aid Certification.
- Fluency in other languages is an asset.
- On-call duties are required.
- This position requires full vaccination against COVID-19, and proof of vaccination must be provided prior to employment.
