About the Role
We are seeking a skilled Registered Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality patient care in various settings, working with patients of all ages and complexity of care needs.
Key Responsibilities
- Conduct thorough assessments of patients' individualized needs using appropriate tools and techniques.
- Develop and implement care plans, continuously monitoring and updating patient records.
- Engage patients using a collaborative approach, building rapport and trust.
- Provide reliable and flexible care, with a willingness to travel to and from patient residences within a designated area.
Requirements
- Registration with the CNO as a Registered Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in delivering community care is an asset.
