  • Collaborate with colleagues to coordinate patient care, ensuring seamless transitions and optimal outcomes.
  • Provide education and support to patients and their families, empowering them to take an active role in their care.
  • Maintain accurate and detailed records of patient interactions, care plans, and outcomes.
Requirements:
  • Licensure as a Registered Nurse by provincial or territorial authorities.
  • A minimum of 1 year of experience in a hospital or home for the aged setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong problem-solving and critical thinking skills, with a focus on delivering exceptional patient care.
  • A willingness to relocate and participate in a fast-paced environment.
