Nurse Care Coordinator

Tofield Continuing Care is a 50-bed unit located in a rural setting. As a Licensed Practical Nurse, you will work within an interdisciplinary team environment to provide client-centered care.
The LPN must be able to work to their full scope of practice, providing leadership and direction to unregulated staff under the supervision of the Nurse in Charge. This role requires critical thinking skills to plan, assess, implement, and evaluate client-centered care.
Our Ideal Candidate- A strong interest and skills in providing care to elderly clients, including those diagnosed with Alzheimer Disease and other dementias.
- An interprofessional healthcare team utilizing nursing processes, critical thinking, problem-solving, and decision-making skills.
You will be guided by your education, experience, demonstrated skill level, and AHS policies and procedures. Your role will be key in providing safe, quality patient and family-centered care while reflecting the shared vision and values of AHS.
Requirements- Completion of an accredited practical nurse education program.
- Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
You will need to possess:
- Relevant technical nursing skills.
- Basic computer skills.
- Good communication skills and the ability to maintain effective interpersonal relationships.
- Demonstrated problem-solving skills.
- Ongoing responsibility for maintaining own continuing competence through professional development activities.
- Able to work shift work.
- Able to meet the physical demands of the position.
- Recent gerontology nursing experience and/or education.
- Experience in a continuing care environment within the last three years.
- Experience with the RAI-MDS.
- Experience using the Connect Care system.
