The Registered Nurse is a key member of the VON Canada team, responsible for providing high-quality, client-centered care to individuals and families in their homes and community. As a Registered Nurse, you will work collaboratively with clients, families, and healthcare teams to develop and implement personalized care plans that promote health, independence, and well-being.
Key Responsibilities- Assess client needs and develop comprehensive care plans that address physical, emotional, and social needs.
- Provide direct care to clients, including medication management, wound care, and mobility assistance.
- Collaborate with healthcare teams to coordinate care and ensure seamless transitions between healthcare settings.
- Provide education and support to clients and families to promote health literacy and self-management.
- Conduct regular assessments and evaluations to ensure client needs are being met and care plans are effective.
- Document client information and care plans in a timely and accurate manner.
- Participate in quality improvement initiatives to enhance client care and services.
- Current registration with the College of Nurses of Ontario.
- Canadian Nurses Association certification in Community Health Nursing an asset.
- Experience in community health nursing an asset.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Ability to prioritize tasks and manage time effectively.
