At Homewood Health, we offer an exciting opportunity for a Clinical Lead Therapist to join our Trauma Services team. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the development of our trauma services.
About the RoleIn this critical position, you will be responsible for conducting specialized assessments, developing treatment plans, and coordinating patient care using Occupational Therapy intervention strategies. Your expertise will help individuals overcome mental health challenges and regain control over their lives.
Key Responsibilities- Assessment and Treatment Planning: Conduct comprehensive functional, cognitive, and return-to-work readiness assessments to inform treatment plans.
- Patient Care Coordination: Develop and implement individualized treatment plans, ensuring seamless integration of care.
- Interdisciplinary Collaboration: Work closely with healthcare professionals to ensure holistic patient care.
- Trauma Support: Provide individual therapy sessions and facilitate group programs to support patients in their recovery journey.
- Caseload Management: Prioritize and manage your caseload effectively, ensuring timely and quality care delivery.
- Documentation and Reporting: Maintain accurate and timely documentation and reports as per COTO standards and referral expectations.
- Educational Requirements: Graduate of a recognized Occupational Therapy Program required.
- Licensure and Certification: Member of the College of Occupational Therapists of Ontario required.
- Experience: Demonstrated experience working with individuals with PTSD, mood disorders, anxiety disorders, substance use disorders, and various comorbidities.
- Leadership and Communication Skills: Proven leadership and communication skills in interdisciplinary teams essential.
