Responsibilities:

  • Conduct comprehensive case reviews and respond to specific questions from service providers to optimize pain and symptom management for clients.
  • Design and deliver educational sessions, workshops, and in-services tailored to meet the needs of front-line providers and promote organizational objectives.
  • Foster connections between organizations, professionals, and evidence-based resources to facilitate seamless access to quality care.
  • Engage in collaborative efforts with various hospice palliative care committees focused on planning, clinical issues, advocacy, and integration with other healthcare sectors.
  • Spearhead palliative care education initiatives aimed at enhancing the skills and certifications of healthcare professionals.
  • Participate in regional strategic planning exercises and contribute to the development of effective policies and procedures.
  • Monitor and evaluate progress towards program objectives, ensuring alignment with MIS-OHRC standards.
  • Liaise with stakeholders to facilitate smooth accreditation processes.
  • Develop and refine best practices in palliative care.
  • Collaborate in research projects to advance the field of palliative care.
  • Adhere to principles of neutrality and impartiality in advisory roles.

Essential Qualifications:

  • RN/RN(EC) membership with College of Nurses of Ontario (CNO) in good standing.
  • Accreditation in hospice palliative care or equivalent experience.
  • CNA certification, including CHPCN(C), CON(C), and CCHN(C).
  • A minimum of 5 years' experience in hospice palliative care, with current expertise in palliative and end-of-life pain and symptom management.
  • Background in program planning, PDSA cycles, and Continuous Quality Improvement.
  • Comprehensive knowledge of healthcare systems.
  • LEAP Co-facilitator certification and LEAP training as assets.
