Nurse Director

VON Canada is seeking a skilled Registered Nurse to fill the role of Directing Registered Nurse (DRN). As a DRN, you will be responsible for managing pediatric, palliative, and acute medically complex clients with Chronic Obstructive Pulmonary Disease (COPD) or Congestive Heart Failure (CHF) by directing Personal Support Workers (PSWs) and providing clinical direction and assignment of tasks within their practice statement.
Responsibilities- Provide remote assessment and management of care services to clients in their homes
- Collaborate with the multidisciplinary healthcare team to ensure all patient care needs are met
- Maintain accurate client clinical records and patient assignment within the virtual ward
- Communicate effectively with team members, outside service providers, and case managers as needed
- Participate in CNO Reflective Practice and uphold VON standards and eHomecare processes
- Registered and in good standing with the College of Nurses of Ontario
- DRNs must hold a CNA Certificate in Palliative Care or have completed both the "Fundamentals of Palliative Care" and the "Comprehensive Advanced Palliative Care Education (CAPCE)"
- Pediatric DRNs must have extensive hands-on experience in pediatric care
- Recent palliative, pediatric, and/or acute medical experience with COPD and CHF patients required
