Physiotherapist - Clinical Mentor

Job Summary
Vancouver Coastal Health is seeking a Physiotherapist to join the Solid Organ Transplant (SOT) team at Vancouver General Hospital, working as a Clinical Mentor. The successful candidate will work in a team-based environment and provide therapeutic interventions to designated clients/residents.
Key Responsibilities
- Assess clients'/residents' physiotherapy needs, functional abilities, and goals, establishing a physiotherapy diagnosis and prognosis.
- Develop and implement an intervention strategy, evaluating the effectiveness of the intervention, and communicating with the client/family and healthcare team members.
- Implement physiotherapy care plans, evaluate the effectiveness of the intervention, and communicate with the client/resident/family and healthcare team members.
- Support the Practice Coordinator and Educator and participate in program development and evaluation, focusing on mentorship and education services.
- Consult, confer, and collaborate with program and team members as well as with the Practice Coordinator in promotion of continuous quality improvement, implementation of interdisciplinary research, and in coordinating activities required for progressive educational and clinical practice programs.
Qualifications
Education & Experience
- Degree in Physical Therapy from a recognized program.
- Current full registration with the College of Physical Therapists of BC (CPTBC).
- Additional three (3) years of recent, related experience, including one (1) year's related mentoring, preceptoring, coaching, and/or group facilitation experience, or an equivalent combination of education, training, and experience.
- Eligible for membership in the Physiotherapy Association of British Columbia (PABC).
Knowledge, Skills & Abilities
- Broad knowledge of Physiotherapy theory and practice within a client and family-centered model of care.
- Broad knowledge of the CPTBC standards of practice and guidelines for clinical practitioners.
- Basic knowledge of physiotherapy standardized assessments, tests, and measures.
- Knowledge and demonstrated skill in the provision of evidence-based clinical practice related to designated client populations.
- Knowledge of provincial acts and regulations related to home and community care.
- Knowledge of acute, chronic disease, palliative, and health management and self-management support.
- Knowledge of community resources available for client/family support and related health services.
- Knowledge of transitions through continuums of care.
- Knowledge of other healthcare professions and their role in client care.
- Knowledge of research process and methodology.
- Knowledge of adult education principles, methods, and tools.
