Job Summary

We are seeking a skilled Physiotherapist to join our dynamic Rehabilitation Team in the role of Physiotherapist. As a Physiotherapist with CarePartners, you will play a critical role in providing high-quality patient care, assessing patients in the community and in their homes, and developing effective care plans. You will work collaboratively with our interdisciplinary care team to provide active guidance and support to patients, promoting independence and well-being.

Key Responsibilities
  • Conduct professional assessments of patients in the community and in their homes
  • Develop and maintain effective care plans in collaboration with the Home and Community Care Support Services (HCCSS)
  • Work with patients to set and achieve time-specific, measurable goals and outcomes
  • Provide one-on-one therapeutic support to clients, promoting independence and well-being
  • Participate in effective treatment programs and provide active guidance to patients
Requirements
  • Master's Degree (or equivalent) in Physiotherapy from a recognized university
  • Registration with the College of Physiotherapists of Ontario
  • Proven computer skills, including email, word processing, and work scheduling systems
  • Strong assessment skills and ability to participate in effective treatment programs
  • Excellent English verbal and written communication skills
  • French language skills are an asset
