Carewest Innovative Healthcare is seeking a highly skilled and dedicated individual to fill the position of Long Term Care Specialist. This role is a key part of our mission to provide exceptional care to our clients and support them in maintaining their quality of life as they transition through their healthcare journey.
Key Responsibilities- Lead clinical team consultation and collaborate with stakeholders to implement system change and improve long-term care nursing practice.
- Develop and implement clinical guidelines and protocols to ensure high-quality patient care.
- Identify opportunities to increase long-term care nursing practice based on best evidence and implement initiatives to improve patient outcomes.
- Collaborate with healthcare professionals to promote excellence in nursing and advance clinical care.
- Completion of an accredited Registered Nurse education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) certification.
- Minimum 3 years of full-time clinical experience in continuing care, focusing on long-term care.
- Previous CCHSS auditor experience and/or LTC educator experience.
- Completion of Master's in Nursing.
- PROSCI Change Management certificate.
- Alberta Health Services Improvement Way (AIW) Green Belt certification.
- Canadian Gerontological Nursing certificate (GPNC).
- Project management and change management knowledge and experience.
