Job Summary


The University Health Network seeks a skilled Clinical Social Worker to join our Spinal Cord Rehab Program at the Toronto Rehab Institute - Lyndhurst Centre. This is a temporary full-time opportunity approximately 12 months.



Key Responsibilities



  • Assess patients to evaluate their need for service and formulate treatment plans.
  • Implement intervention plans with patients, family members, and the interprofessional team to facilitate changes in behavior and attitudes.
  • Facilitate family/team meetings and reassess and modify plans as required.
  • Liaise with community agencies and complete applications for Alternate Level of Care facilities.
  • Render services for the immediate benefit of the Hospital and the community.
  • Provide clinical teaching to Social Work Students and Medical Students.
  • Participate in relevant research activities and perform activities required for the effective operation of the Social Work professional group.


Requirements



  • Completion of a Masters Degree in Social Work.
  • Registration with the Ontario College of Social Workers and Social Service Workers.
  • Membership in the Ontario Association of Social Workers, preferred.
  • Experience working with an interprofessional team in a healthcare setting, preferred.
  • Excellent verbal and written communications skills.
  • Excellent interpersonal, time management, and organizational skills.
  • Excellent problem-solving skills.
  • Research experience/education; an asset.
  • Student education/supervision; an asset.
  • Knowledge of policies, legislation, programs, and issues related to social welfare institutions and services.
  • Computer literacy, preferred.
  • Knowledge of a second language; an asset.
