Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team as a Hospice Care Specialist. In this role, you will provide safe and respectful nursing care to patients with complex or unpredictable outcomes, working collaboratively with the RN/RPN and other members of the interdisciplinary care team. Your expertise will be essential in supporting patients and their families through the end-of-life journey, promoting dignity and comfort, and advocating for their needs.

Responsibilities
  • Conduct thorough assessments, monitor patient status, and communicate with the RN/RPN and other team members as required.
  • Administer medications, including narcotics, according to established procedures, and monitor their effects.
  • Document patient care, including assessments, status, progress, treatments, changes in condition, medications administered, and discharge planning.
  • Collaborate with the interdisciplinary team to develop, modify, and evaluate individualized care plans that support patients' abilities, preferences, and routines.
  • Participate in staff meetings, special projects, and relevant hospital committees to provide feedback on patient care and nursing practice.
Requirements
  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
  • Current basic cardiac life support certification.
