Patient Care Coordinator

Job Summary
The Patient Care Coordinator, Outpatient Cancer Clinics and Systemic Therapy Suite, is responsible for planning, coordinating, and assessing all aspects of care for cancer clients and their families.
Key Accountabilities:
• Assess cancer clients and develop individualized care plans
• Collaborate with oncologists and other healthcare professionals to deliver seamless patient care
• Stay current on best practices and evidence-based care standards
• Contribute to a positive and respectful work environment
Required Skills and Qualifications:
To succeed as a Patient Care Coordinator, you will need:
• A registered nurse diploma or degree
• Strong clinical skills and knowledge of cancer care
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Self-motivation and a passion for delivering exceptional patient care
