Job Summary

We are seeking an Advanced Practice Provider (APP) to join our established Primary Care practice. As an APP, you will be responsible for performing patient assessments, assigning diagnoses, and implementing treatment plans in accordance with current scientific research and standards of care. You will also provide education and counseling to patients, families, and groups, as well as participate in ongoing professional development to stay up-to-date on the latest advancements in primary care.

Responsibilities

  • Perform patient assessments and assign diagnoses based on recorded assessment data and current scientific research.
  • Establish plan of care and implement interventions in accordance with care paths, protocols, standards of care, and current research.
  • Provide education and counseling to patients, families, and groups.
  • Clinic hours are Monday – Friday 8am-5pm.
  • No call or weekends.
