Job Overview

Fraser Health Authority is seeking an experienced Indigenous Social Worker to join our team as a Primary & Community Care Social Worker in Hope, Agassiz, Harrison Hot Springs, and Kent. This full-time position offers the opportunity to work with individuals and families who identify as Indigenous, promoting self-determination and delivering high-quality care.

About the Role

This exciting role will involve providing comprehensive services and therapeutic clinical counseling to clients and their families. You will conduct comprehensive social work and psychosocial assessments, utilizing various assessment tools to provide client treatment services, including liaising with community agencies and caregiver support groups. Additionally, you will participate on local committees and planning bodies to identify areas for improving service delivery and reducing service gaps.

Duties and Responsibilities

  • Conduct comprehensive social work and psychosocial assessments by interviewing clients and/or their families, obtaining relevant information, gathering social data, and interpreting the data using various tools, in accordance with professional standards, clinical policies, legislation, and culturally appropriate Indigenous practices.
  • Provide comprehensive therapeutic services by exploring the psychosocial response to identify problems, personal concerns, treatment objectives, and facilitating change in coping style, behavior, attitude, and feelings; participate with the client, families, community supports, and the interdisciplinary team to coordinate services, manage cases, and facilitate meetings.
  • Collaborate with the interdisciplinary team in client care planning by interviewing the client and/or family to assess and determine care plan and type of service required; follow up and provide ongoing monitoring of complex psychosocial situations; evaluate clients' responses to interventions.
  • Support a seamless approach to care and service delivery across the healthcare system by participating in the admission and discharge process of clients in acute care and community; arrange meetings to assist in developing and coordinating comprehensive care plans and community services for clients; collaborate with the hospital and community services to determine the effective utilization of available resources.

Requirements

  • Current full registration with the BC College of Social Workers.
  • Two (2) years' recent related experience in a complex health care environment such as a community or home health setting, plus relevant related experience in an Indigenous environment/community; or an equivalent combination of social work education, training, and experience.
  • Valid BC Driver's License and access to personal vehicle for business-related purposes.
