Job Summary
We are seeking a skilled Community Health Worker II to join our team at Northern Health Authority. As a key member of our interprofessional healthcare team, you will provide home support services to clients in accordance with a pre-established care plan. Your primary responsibilities will include assisting with activities of daily living, promoting maximum independence, and ensuring the well-being of individuals and families.

Key Responsibilities
• Provide home support services to clients in accordance with a pre-established care plan
• Assist with activities of daily living, such as bathing, grooming, and oral hygiene
• Promote maximum independence and well-being of individuals and families
• Perform delegated tasks for which delegation of tasks training has been completed
• Establish and maintain rapport with clients, family, and care team
• Communicate verbally and in writing with clients, family, and care team

Requirements
• Graduation from a recognized Home Support Attendant/Resident Care Aid Program
• Registration with the BC Care Aid & Community Health Worker Registry
• Valid Class 5 BC Drivers' License
• Certificates in CPR, First Aid, and Food Safe
• Use of personal vehicle
• Or equivalent combination of current education, training, and experience
