About the Role:

We are seeking a highly skilled Patient Care Coordinator to join our team at Health Careers in Sask. As a Patient Care Coordinator, you will play a critical role in providing case management services to clients, promoting quality care and cost-effective outcomes.

Job Summary:

The successful candidate will be responsible for following assigned clients from admission to discharge, providing assessment, coordination, implementation, monitoring, and evaluation of health-related services. You will maintain a collaborative client-centered approach, working across provider and agency boundaries to promote the client's expressed health goals and general well-being.

Key Responsibilities:

  • Provide case management services to clients, including assessment, coordination, and implementation of health-related services

  • Promote quality care and cost-effective outcomes, with the client's expressed health goals and general well-being at the forefront
  • Work independently, using your knowledge and skills to make informed decisions

Requirements:

  • Bachelor of Social Work (BSW) degree or equivalent
  • Valid Class 5 driver's license
  • Bachelor of Nursing is acceptable
  • Basic interpersonal, conflict resolution, writing, verbal communication, organizational, and problem-solving skills
  • Knowledge of community resources and services in the clinical area
  • Ability to complete case management assessments, follow-up, and treatment

Preferred Qualifications:

  • Registered/eligible for registration with the appropriate licensing body

Compensation:

The salary for this position is estimated to be between $36.72 and $44.90 per hour, based on the Pay Band Assessor Coordinator - Degree rate. The actual salary may vary depending on factors such as location and experience.
