Key Responsibilities- Participate in scheduled visits to assess patient needs and provide care within your scope of practice.
- Provide safe and effective nursing care, using appropriate assessment tools and techniques to identify individual patient needs.
- Develop and implement care plans, monitoring patient progress and adjusting plans as required.
- Build strong relationships with patients and their families, using a collaborative approach to care.
- Maintain accurate and up-to-date patient records, documenting all patient encounters and care plans.
- Registration with the College of Nurses of Ontario (CNO) as a RPN in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a willingness to travel to patient residences within a designated area.
