About the Role

We are seeking a Clinical Pharmacist to join our team at Mackenzie Health. This is an excellent opportunity for a healthcare professional to work in a dynamic and multidisciplinary environment.

Job Summary

The successful candidate will contribute to overall desired patient outcomes through provision of pharmaceutical care, including drug therapy monitoring, pharmacokinetic consultation, and drug information services. They will also play a key role in ensuring safe, quality care by assessing, planning, implementing, and evaluating drug therapy effectively.

Key Responsibilities
  • Assess patients' medication needs and develop personalized treatment plans.
  • Provide education to patients about medications to promote autonomy and reduce non-compliance.
  • Apply the pharmaceutical care model to identify, resolve, and prevent drug-related problems.
  • Conduct drug utilization reviews to ensure cost-effective usage of medications.
  • Co-ordinate the acquisition and supply of in-center and home medications.
  • Participate in formulating the interdisciplinary plan of care.
  • Provide effective supervision, leadership, and direction to Pharmacy Technicians, students, and volunteers.
Requirements

To be successful in this role, you will require:

  • A Bachelor Degree in Pharmacy from a recognized institution.
  • Licensure with the Ontario College of Pharmacists in Part A of the Register (or eligibility for registration).
  • Previous hospital experience.
Preferred Qualifications

While not required, the following qualifications would be an asset:

  • Hospital residency.
  • PharmD degree.
  • Demonstrated pharmaceutical care, communication, and computer skills.
