Job Summary

We are seeking a highly organized and detail-oriented individual to join our team as a Team Assistant. As a key member of our Care Coordination team, you will provide administrative and clerical support to ensure the smooth delivery of patient care services.

Key Responsibilities
  • Provide information and referral services to patients, families, and service providers, ensuring a high level of client satisfaction.
  • Assist Care Coordinators with operational functions, including data entry, document management, and communication with stakeholders.
  • Maintain accurate and up-to-date client records, ensuring confidentiality and compliance with established guidelines.
  • Process requisitions, prepare related documents, and monitor inventory of supplies.
  • Book client appointments, prepare agendas, and reserve meeting rooms as directed by Care Coordinators.
  • Arrange client transportation and maintain team schedules.
Requirements
  • High school graduation diploma plus one year of Community College diploma in Office Administration, Business, or Health Administration.
  • Minimum 18 months of experience in customer service or a related position in the health, social, or business administration fields.
  • Demonstrated knowledge of office procedures, computer software applications, and effective communication skills.
