About the Role: As a Licensed Practical Nurse with ParaMed Home Health Care, you will play a vital part in delivering high-quality community nursing services. You will visit patients in their homes, providing expert care and support to help them maintain independence and comfort.

Your Responsibilities:

  • Conduct scheduled visits to assess patient needs and provide personalized care
  • Collaborate with RNs to ensure comprehensive care for complex patients
  • Use assessment tools and techniques to identify individual patient needs
  • Develop and implement effective care plans, monitoring progress and adjusting as necessary
  • Build strong relationships with patients and families through empathetic communication and trust-building
  • Maintain accurate and up-to-date patient records

What We're Looking For:

  • Current registration with the CLPNA as an LPN with active status
  • A valid driver's license and reliable access to a vehicle
  • Experience in community care is an asset, but not required
  • Reliability, flexibility, and a willingness to travel within a designated area
  • Passion for making a positive impact in your community
