Job Description

The Community Care Clinician, ACT Services will work as a member of the interdisciplinary treatment team, providing advanced clinical practice and interventions in the community setting. The successful candidate will have a Master's degree in a health-related discipline and three years of recent, related experience.

Key Responsibilities

  • Screen and respond to referrals through the intake process
  • Provide mental health and/or substance abuse assessments
  • Develop service plans that maximize rehabilitation and utilize relevant community supports
  • Participate in discharge planning and follow-up

Requirements

  • Master's Degree in a health-related discipline from an approved post-secondary institution
  • Three (3) years of recent, related experience, or an equivalent combination of education, training, and experience
  • Demonstrated ability to communicate effectively, both verbally and in writing
