About the Role

We are seeking a highly skilled and compassionate social worker to join our team as an acute care social worker. This role will involve providing comprehensive therapeutic clinical counselling services to patients and families in various clinical specialty areas within acute care.

Key Responsibilities

">
  • ">
  • Provide grief and bereavement support to patients and families;">
  • Conduct comprehensive social work and psycho-social assessments of patients and families;">
  • Educate and consult with patients, families, and healthcare professionals on issues related to high-risk factors, recovery, and community practice;">
  • Maintain accurate patient records and participate in discharge planning and hospital stay coordination;">
  • Communicate effectively with community health and social agencies;">
  • Participate in program evaluation and identify opportunities for change;">
  • Develop and deliver in-services, presentations, psycho-educational groups, and training related to assigned clinical specialty areas;">
  • Initiate, develop, and carry out research activities relevant to patient, family, hospital, and community needs.">

Requirements

  • ">
  • Bachelor's or Master's degree in Social Work from an accredited school;">
  • Current full registration with the BC College of Social Workers;">
  • Two years' recent experience in social work, preferably in acute care settings;">
  • Excellent communication, organizational, and leadership skills;">
  • Able to operate related equipment and maintain physical ability to perform job duties.">
