Nurse Practitioner

ComForCare Home Health Care - Kitchener/Waterloo is seeking a skilled Nurse Practitioner to join our team. As a Comprehensive Care Specialist, you will be responsible for conducting thorough client assessments, creating personalized care plans, and implementing interventions to achieve client goals.
Key Responsibilities:- Perform comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
- Develop an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
- Provide interventions with a focus on achieving realistic client outcomes.
- Report changes to update the plan of care to reflect progress towards goals and outcomes.
- Administer medications as needed.
- Maintain client records showing systematic assessments, planning interventions, and evaluations.
- Initiate referrals as needed to other health team members.
- Graduate of, at a minimum, a Practical Nursing Program (2-year college certificate)
- Registered with the College of Nurses
- Minimum of one (1) year experience in an acute care setting or equivalent experience
- CPR certified
- Demonstrated written, verbal, and interpersonal communication skills
- Licensed driver with automobile insured in accordance with provincial and/or Agency requirements
- Competitive salary range: $24/hr - $25/hr
