**About the Role**

BAYADA Home Health Care is a leading provider of skilled nursing and rehabilitative care in the community. We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in Concord, NH.

The ideal candidate will have a minimum of one year of recent experience as an LPN and be able to work independently with minimal supervision. Strong communication and interpersonal skills are essential for this role.

Responsibilities:

  • Making home visits to clients living in designated territories
  • Performing assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager
  • Following up with, executing, and properly documenting doctors' orders
  • Performing client assessments as necessary
  • Case management and coordination
