About the Role:
We are seeking a skilled Registered Practical Nurse to join our team in a community nursing role. As a RPN, you will provide high-quality care to patients in their homes, working collaboratively with RNs to support complex patients.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Use assessment tools and techniques to identify individual patient needs and develop care plans
- Provide safe and ethical nursing care, engaging patients through a collaborative approach
- Maintain accurate patient records and update plans as required
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in community care is an asset
- Reliability, flexibility, and a willingness to travel to patient residences
- Passion for making a difference in your community
