Job Summary

Vancouver Coastal Health is seeking a skilled Social Worker to join the Integrated Care Team at the Downtown Community Health Centre. As a key member of the team, you will work collaboratively to provide comprehensive care to clients and families experiencing health challenges.

Key Responsibilities

  • Provide client-centered care, addressing physical, emotional, and social needs
  • Collaborate with interdisciplinary teams to coordinate care and support transitions between services
  • Conduct mental status exams and suicide risk assessments
  • Develop and implement care plans, providing supportive counseling and crisis intervention as needed
  • Act as a resource for the team, providing information and support related to clients with complex health care needs
  • Establish and maintain therapeutic relationships based on respect and trust

Qualifications

  • Master's Degree in Social Work or related field, plus two years' recent experience in a similar role
  • Current registration with the British Columbia College of Social Workers
  • Valid BC Driver's License (preferred)

Knowledge and Skills

  • Comprehensive knowledge of mental health, substance use, and primary care principles and practices
  • Ability to work effectively in a fast-paced, dynamic environment
  • Strong communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders
  • Ability to provide care coordination to a select caseload of clients
  • Knowledge of crisis intervention and supportive counseling techniques
  • Ability to work independently and as part of a multidisciplinary team
