The Case Manager plays a crucial role on a cross-functional team responsible for delivering clinical support and case management for members with mental health challenges. This individual ensures high-quality care and exceptional member satisfaction by working directly with the member to develop and measure progress against action plans.

  • Provide best patient care through video and phone interactions
  • Assess patients' mental health concerns and perform counseling
  • Support Mental Health Care and Employee & Family Assistance Programs

Key Responsibilities:

  • Serve as case manager for members during program participation, ensuring the highest possible member experience and proactive communication
  • Obtain comprehensive health history through intake assessment
  • Facilitate expedited procurement of member medical records
  • Assist members in implementing treatment, identifying best path to care
  • Provide technical and case support to Teladoc clinicians

**Requirements**

  • Bachelor's or Master's Degree in Social Work, Nursing, or Occupational Therapy
  • 5+ years' experience in a professional office environment
  • Bilingual fluency (English and French)
