Alberta Health Services Central Zone is seeking a qualified Social Worker II to join their Transition Services and Seniors Health Portfolio. As a key member of the inter-disciplinary team, you will provide family and patient-centered care across the continuum of care, including Acute Care, Home Care, Supportive Living, and Long-Term Care.

Key Responsibilities:
  • Provide psychosocial services using a collaborative approach to support clients, families, and health professionals.
  • Conduct psychosocial assessments, comprehensive risk assessments, and cognitive screening.
  • Support transitions and discharge planning, and provide education to clients and their families.
  • Collaborate with multi-disciplinary teams to provide family and patient-centered care.
Required Qualifications:
  • Completion of a bachelor's degree in Social Work.
  • Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Preferred Qualifications:
  • Excellent communication skills.
  • Experience working within an interdisciplinary team.
  • Knowledge of relevant legislative Acts and experience with guardianship, trusteeship, personal directives, and capacity assessment processes.
  • In-depth knowledge and experience in working with Income Support programs at a community/provincial/federal level.
