The successful candidate will be responsible for identifying patients suitable for the bundled care pilot "Southlake@Home" and acting as a liaison between home and community care providers and the Southlake hospital care team.

The Clinical Care Coordinator will collaborate with patients, physicians, caregivers, family members, and care-team members to ensure seamless care transitions and efficient use of resources.

Key responsibilities include:

">
  • Liaising with community providers on a daily basis;
  • Participating in joint problem-solving and case management to ensure provider interventions meet patient goals;
  • Referring patients to CLHIN home care services when necessary;
  • Facilitating transitions to the CLHIN home and community care program;
  • Providing information on community resources and facilitating referrals to Community Support Services;
  • Maintaining knowledge of Quality Improvement methodologies and implementing best practices guidelines.

To succeed in this role, you will need:

  • Strong communication and interpersonal skills;
  • Ability to work collaboratively with healthcare professionals and community agencies;
  • Knowledge of Quality Improvement methodologies and experience working on clinical pathways and best practice guidelines;
