Clinical Pharmacist

**Education and Training:**
We are seeking a licensed pharmacist with a strong educational background and relevant training in clinical pharmacy practice.
**Experience:**
Applicants must have a minimum of two years' recent experience as a pharmacist in a hospital setting, providing direct patient care, or an equivalent combination of education, training, and experience.
**Skills and Abilities:**
- Ability to identify and resolve complex drug therapy problems.
- Ability to apply clinical knowledge and make sound decisions with minimal supervision.
- Ability to work independently and as part of a multidisciplinary healthcare team.
- Ability to establish and maintain effective working relationships with physicians, other healthcare professionals, patients, and family members.
- Ability to communicate effectively both verbally and in writing.
- Ability to be self-directed, organize and prioritize workload, and complete tasks with accuracy, precision, and dependability.
- Ability to teach and educate patients and healthcare professionals.
- Ability to operate related equipment and use standard office software.
- Ability to physically perform the duties of the position.
As a Clinical Pharmacist with Island Health, you will be responsible for providing direct patient care, including:
- Conducting medication therapy reviews and making recommendations for optimization.
- Developing and implementing individualized patient care plans.
- Collaborating with healthcare teams to ensure safe and effective medication use.
- Providing education and support to patients and families.
