The estimated annual salary for this position is $85,000-$110,000, depending on experience and qualifications.
Job Description:
We are seeking a highly skilled Registered Practical Nurse (RPN) to join our team as a Clinical Care Coordinator - Geriatric Nursing Specialist. In this role, you will be responsible for enriching the lives of residents by ensuring they can make their own choices and lead full lives as independently as possible. You will work under the direction of the Health & Wellness Manager to develop and implement resident care plans, supervise care given by attendants, and provide direct resident care. Your excellent communication skills and ability to work well with others will be essential in maintaining a positive and supportive team environment.
Key Responsibilities:
- Participate in assessing the needs of individual residents and organize plan of care to promote maximum resident independence.
- Ensure the resident's file is established, with proper documentation and safe practices related to medication administration processes.
- Distribute medications and provide treatments to residents in accordance with current and approved standards of care, policies and procedures.
- Provide direct resident care, in accordance with current and approved standards of care, policies and procedures.
Required Skills and Qualifications:
To succeed in this role, you must have:
- A minimum of grade 12 education required.
- A recognized nursing license (RPN licensing) and in good standing with College of Nurses of Ontario (CNO).
- Certified in Emergency First Aid, CPR and AED certifications.
- Previous experience working as an RPN in a health care setting is considered an asset.
