Job Title: Nurse Practitioner

St. Joseph's Healthcare Hamilton is seeking a skilled Nurse Practitioner to join our Schizophrenia Outpatient Clinic and Transitional Outpatient Program for Schizophrenia Service.

Job Summary:

The successful candidate will work in collaboration with physicians, other members of the interdisciplinary team, and clients/families to provide evidence-informed and integrated care to registered outpatients of the TOPSS and SOC. The Nurse Practitioner will utilize a clinical case management model of service delivery, striving to support individuals with schizophrenia and experiences with psychosis to regain and manage their health.

Key Responsibilities:
  • Provide autonomous health care to registered outpatients of TOPSS and SOC
  • Collaborate with physicians and other members of the interdisciplinary team
  • Develop and implement individualized care plans
  • Provide education and support to clients and families
  • Participate in quality improvement initiatives
Requirements:
  • Current registration with the College of Nurses of Ontario in the RN-Extended Class with specialty certification as NP - Primary Health Care
  • Master's Degree in Nursing
  • Proof of successful completion of controlled substances education
  • Faculty appointment or eligibility for appointment with a School of Nursing in an academic institution
  • CNA certification in Psychiatric/Mental Health Nursing (CPMHN(c)) preferred
Preferred Qualifications:
  • Demonstrated experience with psychiatric assessment
  • Providing pharmacologic and non-pharmacologic interventions
  • At least three years recent demonstrated experience as a Nurse Practitioner with experience in community-based settings servicing individuals diagnosed with schizophrenia and related psychotic disorders
