About the Role

We are seeking a skilled and compassionate Community Health Nurse to join our team in Burnaby, BC. As a key member of our interprofessional team, you will work independently in the community setting, assessing and treating clients with complex health conditions.

Key Responsibilities:
  • Conduct thorough assessments of clients' physical and emotional needs, identifying opportunities for health promotion and disease prevention.
  • Develop and implement comprehensive care plans that address clients' unique needs and goals, collaborating with primary care providers and other healthcare professionals as needed.
  • Provide direct client care, including education, coaching, and support to empower clients to manage their chronic conditions effectively.
  • Collaborate with clients, families, and caregivers to identify and address barriers to care, ensuring seamless transitions between healthcare services.
  • Maintain accurate and up-to-date clinical records, tracking patient outcomes and contributing to quality improvement initiatives.
