Key Responsibilities

  • Conduct home visits to assess client needs and develop individualized care plans
  • Collaborate with healthcare professionals, family members, and community agencies to ensure seamless care delivery
  • Manage and coordinate resources and services to meet client needs
  • Develop and implement programs to promote healthy aging and wellness

Qualifications

  • Post-secondary education in a related field (e.g., gerontology, social work)
  • Minimum 3 years of experience in a similar role
  • Strong understanding of Indigenous cultures and traditions
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
