In this dynamic position, you will be responsible for conducting thorough assessments, creating personalized care plans, and coordinating treatment that addresses the unique needs of each client. Working closely with a multidisciplinary team, you will provide direct counseling, crisis intervention, and facilitate group sessions aimed at empowering clients to manage their own care.

Key Responsibilities
  • Conduct comprehensive assessments of clients' needs and develop personalized care plans
  • Provide direct counseling and crisis intervention services to clients
  • Facilitate group sessions to promote client empowerment and self-management
  • Collaborate with multidisciplinary teams to ensure seamless care delivery
Requirements

To be successful in this role, you will require:

  • A Bachelor's Degree in Social Work from an accredited university
  • Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience
  • Current full registration with the British Columbia College of Social Workers
  • Current valid BC driver's license
