About the Role:- As a Senior Living Nurse Specialist, you will be responsible for delivering high-quality patient care in a home setting.
- You will work closely with clients and their families to develop personalized care plans that meet their unique needs.
- Conduct thorough assessments of clients' physical and emotional health to identify areas for improvement.
- Develop and implement effective care plans that address clients' specific needs and goals.
- Administer medications, provide wound care, and perform other medical procedures as needed.
- Monitor clients' progress and adjust care plans accordingly.
- Maintain accurate and detailed records of client interactions and treatment outcomes.
- A current license as a Registered Nurse in Pennsylvania.
- A minimum of one year of recent, verifiable clinical experience.
- Prior home care experience is highly desirable but not required.
